You are responsible for contacting your insurance company to determine the out of pocket costs while receiving treatment and you will be responsible for all fees and payments in the event your insurance company does not reimburse no matter what the reason. I would strongly encourage you to contact your insurance provider and ask: are mental health benefits covered by my insurance? Do I have to meet a deductible before my benefits become activated? What is my deductible? Have I met it yet? Is Michelle Tyson considered in network with you (my insurance company)? What will be my co-pay or co-insurance costs?
I am an out-of-network provider for all other insurance companies. I can provide you with the documentation you will need to submit a claim to your insurance company; however, it is your responsibility to verify payment with your insurance company, as benefits for out of network providers is NOT guaranteed.
Good Faith Estimate
Under the law, Health care providers, under the law, are required to provide clients who do not have insurance or who choose not to use insurance an estimate of their out of pocket costs for medical items and services, which also includes counseling.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as medical tests, prescriptions drugs, equipment and/or hospital fees.
- As a client, make sure your health care provider offers you a Good Faith Estimate in writing at least one business day prior to your medical service or item. You may also ask your provider, or any provider you may choose to work with, for a Good Faith Estimate prior to your scheduled service or item(s).
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call the Colorado Division of Insurance at (303) 894-7490 or (800) 930-3745.
Payment
I accept cash, check or credit card. In addition, some health savings accounts consider psychotherapy an approved expense. Credit card information is needed to book an appointment unless it is an EAP session, which, generally speaking, is at no cost to you. Credit card information will be kept on file for all of those who have co-pays, co-insurance, deductibles to meet, or are private fee clients.
Cancellation and no-show policy
I offer a 24-hour cancellation policy to avoid an $80 session fee for late cancellations or no-shows. Please be mindful: insurance companies do not cover the cost of late cancellations and no-shows. You are responsible for payment.
